Esthetic restorations-composite fillings

Due to consequence of fillings with insufficient marginal seal and untreated cariotic processes teeth can get spoiled. Aesthetic treatment in dentistry means the application of materials, which have so much similar characteristics (form, colour, substance) to the natural teeth that the repairs with them can hardly be noticed. In aesthetic dentistry we make an effort to avoid the use of metals and amalgam fillings. Basically, the dental amalgam consists of mercury, silver, tin and copper. Other metals such as zinc or palladium can also be involved as an additive.

Amalgam fillings have dark colour, therefore are not aesthetic. Using amalgam the mixing ratio must be accurate in order to get a long lasting, massive filling. If the assistant doesn’t prepare the amalgam adequately then the quality of the filling will not be favourable. Opinions vary considerably in connection with the effect of mercury. In my opinion, materials and technologies of these days can totally substitute or even surpass the aesthetically unambiguously obsolete amalgam filling.

The aesthetic composite fillings bond to the tooth structure chemically, that is why they have a long life span as a consequence. Their mechanic properties and wear resistance correspond with those of amalgam. Fillings can be even more aesthetic by means of layering techniques. As the tooth itself consists of different colours, a perfect aesthetic result can only be achieved with the aid of layering techniques using materials of different colour and transparency. With this method it is possible to imitate the natural effects. With the help of aesthetic fillings and inlays dentition can be restored perfectly.

The recession of the gum might occur due to the incorrect technique of toothbrushing mainly or becuase of a long lasting inflammation of the gum. The naked tooth surface might become sensitive for cold as well as for osmotic (e.g.: sweets) stimuli. Also the toothbrushing might induce sensitivity in these cases. This abnormality can be treated without drilling, by means of a special varnish manufactured for this purpose. As a result of the therapy the sensitivity of teeth is taken away.

Esthetic inlay, onlay

Introduction

The proper restorations of teeth can not be done with fillings in all cases, but we make inlays in these circumstances.

When to make an inlay?

In some cases we are not able to establish the so called contact point between two teeth with a simple filling. Due to the incorrect contact food can stuck in and this may result the damage of periodontal tissues as well as the increased susceptibility to caries. In case a cavity is too big, the preparation of an inlay should be more favourable than a filling. From our point of view this kind of treatment has the best outcome under such circumstances.

Which materials can be used to make an inlay?

Different kind of materials (gold, ceramic, composite) can be used for an inlay. The selection of the materials is influenced by a lot of aspects. Among these, the developing galvanic current between two different types of metals as well as financial considerations might play an important roll.

Root canal treatments

As opposed to all nasty experiences and rumours root canal treatment is a procedure, that serves the keeping of teeth within the scope of painless treatment. It’s true that a tooth with pulpitis can be anaesthetized not so easy, but today there are state-of-the-art methodes which have the potention to make any kind of tooth numb.

The patient should not feel any pain during the treatment and the technology of these days gives us far the possibility to manage this. By means of root canal treatment we can save our tooth that is why we can avoid the extraction and the denture.

The advantage of root canal treatment carried out in our office is that with the aid of a rotary endodontic system the therapy of any teeth – even molars – can be accomplished successfully.

To fill up the prepared canal we use a new technique as well, that the root canal can be ideally obturated with. An essential condition of the appropriate root canal filling is the exact determination of the root canal length. For measuring the accurate length of the root canal – instead of the imprecise method done with x-ray we use a special instrument developed for this purpose.

Dental prosthesis

Veneer

A thin layer of ceramic (porcelain), that is glued to the buccal surfaces of front teeth. It combines the philosophy of "minimal invasive technique" (i.e. only a very little amount of sound tooth structure is wasted during the preparation) with the aesthetic concepts.

Porcelain - and special prostheses, all-ceramic crowns

The lifelike aesthetic of crowns and bridges is a basic requirement in exclusive dental offices. Preparing the tooth for a crown can damage the aesthetic especially at the margin of the gum. Moreover the conventional ceramic fused to metal crowns and bridges don't let the light go through and therefore the margin of the gum becomes dark or blue scaled beside the prosthesis. For this reason we apply all-ceramic crowns in case of high aesthetic demands.

Crowns

In case crown of the tooth is damaged so much, or/and conservative therapy is not possible we should make a crown. First we grind the tooth until it gets cone-shaped, take an impression and our technician makes the crown ready. The finished prosthesis covers the visible part of the tooth in the mouth like a cap. Crowns with incorrect marginal seal must be exchanged.

Bridges

Bridges are fixed partial dentures.

Removable dentures

There can be partial or complete dental prostheses. We make removable partial denture (RPD), if the number and location of remaining teeth don't let us to prepare fixed prosthesis. When all the teeth are missing, we can only make complete denture, which replaces all of the natural dentition and associated structures. Under these circumstances we can make implants alternatively.

RPDs with attachments

If the position of the remaining dentition indicates to do RPD, it's possible to use fixed and removable parts at the same time. In these cases the denture base is attached to the abutment tooth by means of a prefabricated device (the so called attachment).

Dowel crown

A metal or composite (carbon or glass fibre post) restoration that replaces the entire coronal portion of a tooth and derives its retention from a dowel extending into a treated (filled) root canal. We can have a perfect solution with the application of glass fibre posts in cases where aesthetic considerations are of great importance.

Implantation (dental implants)

Nowadays implantation has become a routine procedure for replacing the missing dentition. In case number and position of remaining teeth allow to make only removable denture, the possibility of implantation should come up. We are able to make fixed denture with this solution. Of course, implantation is preceeded by serious and detailed anatomic, bone structure and general examinations.
